Medical Care
As a member, you get all of your Medicaid benefits and services, including:
- Primary care provider (PCP) visits
- Immunizations and wellness visits
- COVID-19 testing, treatment, and vaccinations
- Maternity care
- Virtual care options, including telehealth and 24/7 NurseLine
- Hospital care
- Lab and X-ray services
- Home health and personal care services
- Hospice care
- Emergency and urgent care
- Specialty care
- Behavioral health services (Mental Health and Substance Abuse Services)

Pharmacy And Prescription Drugs
Your pharmacy benefits are covered under the state Medicaid program and managed by Gainwell Technologies. If you have questions about your pharmacy benefits, call 833-491-0344.
As a part of your benefits, you receive dental coverage through DentaQuest. For more information, contact DentaQuest at 888-291-3762 (TTY 800-466-7566).

Care Management
Managing your healthcare alone can be hard, especially if you are dealing with many health problems at the same time. If you need extra support to get and stay healthy, we can help. As a member of Anthem, you may have a care manager on your healthcare team. A care manager is a specially trained health professional who works with you and your doctors to make sure you get the right care when and where you need it.

Healthy Rewards
Healthy Rewards is a no-cost, optional program for health plan members. The program encourages you to get the care you need to create a healthy lifestyle and then rewards you for completing healthy activities.
You may be eligible to earn these Healthy Rewards through the Benefit Reward Hub:
Pregnant Women And New Moms
- $10 for completing the combo 10 vaccine before age 2
- Up to $20 when you refill asthma medication
- $10 for completing a lead screening (blood test) before age 2
- $10 for completing child and adolescent wellness visits
- Up to $80 annually for diabetic screenings
- Up to $50 for women’s preventive screenings (breast and cervical cancer and chlamydia)
- Up to $40 for tobacco cessation programs and completion
- Up to $45 for completing an annual health screener and dental visit
- $20 for follow-up with the doctor after a hospital stay
- $20 for screening and testing for members taking antipsychotic medication
