Chronic Condition Special Needs (C-SNP) plans are a type of Medicare Advantage (MA) plan designed to meet the unique needs of people with one or more chronic conditions, including diabetes, end-stage renal disease (ESRD), lung conditions or heart disease.

Am I Eligible For A C-SNP?
You may be eligible for a C-SNP if you have one or more of the following conditions, depending on where you live.
Diabetes mellitus
C-SNPs for diabetes typically cover blood glucose monitoring, insulin management, nutritional training and foot care, as well as vision, hearing, dental care. Your primary doctor, endocrinologist (diabetes specialist), and other providers will work with you to coordinate a personalized plan of care.
C-SNP coverage for diabetes is available in California.
End-stage renal disease (ESRD)
If you have this kidney condition, your care team will include your primary doctor and a nephrologist (kidney-care specialist). You’ll receive personal support, nutritional training, necessary treatments, and prescription drug coverage. Preventive care is also covered, including routine health, dental, vision and hearing screenings.
C-SNP coverage for ESRD is available in California.
Chronic lung disorders
These C-SNPs offer special care for those with lung-related disorders such as chronic obstructive pulmonary disease (COPD), asthma, chronic bronchitis, pulmonary hypertension, or emphysema. A pulmonologist (lung specialist) will coordinate with your primary doctor for treatments, medications, and general health management. These plans also include special coverage for items like oxygen supplies and other medical equipment.
C-SNP coverage for chronic lung disorders is available in California.
CHF and cardiovascular disorders
This C-SNP offers special coverage for chronic venous thromboembolism (CVT), chronic heart failure (CHF), cardiac arrhythmias, coronary artery disease, or peripheral vascular disease. A cardiologist (heart specialist) will work with you and your doctor to determine the best course of ongoing care.
C-SNP coverage for chronic heart failure and cardiovascular disorders is available in California.

Most of our Medicare Advantage C-SNP plans include prescription drug coverage and other valuable benefits with $0 premiums, deductibles and copays. You will still have to pay your Medicare Part B premium.

If you’re not eligible for a C-SNP, we offer other types of SNPs, including Dual-Eligible Special Needs plans for those with both Medicare and Medicaid. Our Medicare Advantage plans also offer similar benefits. You just need to be enrolled in Original Medicare to apply.

Enrollment in an Anthem Blue Cross, Anthem Blue Cross Partnership Plan, Anthem BC Health Insurance Company and Anthem Life and Health Insurance Company plans depend on contract renewal.
