Anthem Blue Cross and Blue Shield in Indiana Recognizes Indiana Healthcare Providers in Inaugural Care Provider Recognition Program

Anthem is celebrating top-performing Indiana care providers, recognizing best-in-class care delivered to Medicare Advantage members.

INDIANAPOLIS – Anthem Blue Cross and Blue Shield in Indiana today proudly named Seymour and Brownstown-based Family Medical Center and Evansville-based Dr. Malek’s Office, in its inaugural Care Provider Recognition Program. Created in partnership with its affiliated health plans across the country, Anthem’s new program celebrates top performing independent and hospital-owned medical groups across the nation, recognizing those who deliver unparalleled patient care. 

These Indiana care providers were selected for recognition based on key health measures for care delivered to Anthem’s Medicare Advantage members. These metrics include preventive care screenings, immunization rates, chronic disease management, medication adherence, and several other health outcomes metrics. 

The program is part of Anthem’s commitment to collaboration and active partnership with care providers to create a positive impact on the health of plan members.
