Improved Plan Benefits for 2025
Incentives for Healthy Behaviors: You can now receive a $50 reward for completing your Annual Wellness Visit. This program is called Health Perks and is administered by Anthem's vendor, Icario. You may receive mail and / or phone calls from Icario about the Health Perks program.
Transportation Benefits: Your plan offers 12 one-way trips to covered medical visits, pharmacy trips, and SilverSneakers locations within 60 miles.
In-Home Support: You have access to 30 hours of care from a personal home helper each year. This helper can provide companionship while assisting with light errands, technology, mobility, exercise, and other activities of daily living, such as meal preparation.
Vision Exam: You will have a $0 copay for routine vision exams.
