Employee Assistance Program (EAP): Providers

Anthem's Employee Assistance Program (EAP) offers quick and easy access to confidential counseling and referral services to help employees and their household members deal with daily work and life challenges.

EAP Network and Services

We provide a national network of more than 40,000 health care professionals. Licensed professionals in five disciplines are eligible for membership:

  • Psychologists
  • Social workers
  • Professional counselors
  • Marriage and family therapists
  • Psychiatric or behavioral health nurse specialists

We also offer clients a variety of services beyond just face-to-face counseling sessions, including adoption information, pet care, child care resources, financial/legal consultations, eldercare referrals and more. For additional information:

Frequently Asked Questions

The American Recovery & Reinvestment Act (ARRA), otherwise known as the Stimulus Package, contains provisions expanding some of the existing Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy requirements.

To accommodate one such provision, Anthem associates will now verify fax numbers each time prior to faxing documents containing Protected Health Information (PHI). PHI will be removed from the fax document. However, if you contact Provider Services and verify your fax number, no verification of each fax sent to you will be required, nor will PHI be removed from the document for a period of one year.

EAP asks that you notify Provider Services immediately when you have any changes in address, telephone or fax numbers. To verify your current information on file, send an email message to EAPProviderNetworks@Anthem.com. Please include your first and last name, licensure, current fax number, as well as your mailing and practice addresses so that we may cross reference. Or, you can call Provider Services at 888-252-2680.

Become an EAP Provider

Anthem's EAP credentialing process examines a number of provider qualifications and requires all providers to be licensed behavioral health professionals in their respective states of practice.


We are currently accepting applications.

Anthem and Carelon Behavioral Health have combined their Employee Assistance Program provider networks as of January 1, 2021. We are accepting Provider applications for the combined network. Below is the link to fill out an application. Also provided is a link to CAQH. Carelon utilizes information from your CAQH profile to complete the credentialing process. Your application will process through credentialing much faster if your CAQH profile is up to date.

EAP Provider Resources

The EAP Provider Center is an online self-service center for contracted EAP providers.

With the Provider Center, you can:

  • Submit Anthem client claims
  • Upload a professional profile photo for the member provider locator
  • Update out-of-office information

To register, you will need:

  • Unique EAP Provider ID (printed on your welcome letter)
  • National Provider Identifier

Anthem EAP can use your Council for Affordable Quality Healthcare (CAQH) records for re-credentialing purposes. Just send us an email at EAPProviderNetworks@Anthem.com with your CAQH# in the subject line, and provide your name and CAQH number in the message.

To prevent delays, make sure your CAQH record is up to date, including your current license, liability insurance and W9. Additionally, set the authorization to grant access to “all healthcare organizations” to allow us to access your information.

New to CAQH?

If you are not currently a CAQH participant but would like to be one, please contact us at EAPProviderNetworks@Anthem.com and use the subject line “New CAQH.” One of our representatives will issue CAQH access for you to begin the process.

Anthem’s EAP Electronic Data Interchange (EDI) allows providers to submit claims and retrieve remittance advices and claim file acknowledgements from the insurance carrier or clearinghouse online.

Payer Codes for EAP

The Payer Code for EAP is EAPBH. This code is different from your state behavioral health plan payer code. You can find more information regarding your state’s behavioral health plan payer codes on our EDI site.

Confirmation and Claims Process
  1.  Client is given provider names and phone numbers.
  2.  Client contacts providers and makes provider selection.
  3.  Client identifies they are covered by EAP and sets up appointment with provider.
  4.  Client calls and supplies EAP with date of first scheduled appointment.
  5.  EAP sends referral and claims forms to provider.
  6.  Provider calls EAP to report first session date.

Note: Providers should confirm whether the session will be managed through EAP or Behavioral Health services at the time the appointment is made.
