Find Preferred Laboratory, X-Ray or High-Cost Imaging Providers

Start by entering your desired location, using the ZIP code, enter the provider type (e.g., laboratory, radiology) or the provider name, and select from the drop down. Click the Recognitions button, select Site of Service, and select Done. Your results will show a list of Site of Service providers. In the List View, you will see the designation, “Site of Service for each Preferred Lab/Pathology/Radiology provider.”

Using Preferred Providers that are part of your plan save you money on copays and other costs such as coinsurance.

The State of Connecticut has identified doctors, hospitals and medical care groups in the state that have verified high-quality outcomes for some of the most common medical conditions and procedures. Doctors and locations proven to meet these high standards are known as Providers of Distinction. When you choose a Provider of Distinction for a listed procedure, you will automatically receive a cash incentive in the mail. Some common care services covered in this program include:

Colonoscopy
Hip replacement & hip revision
Bariatric surgery
Prenatal care and delivery
Lumbar spine fusion

When you are enrolled in the State Preferred, Out of Area, Primary Care Access, Standard Access, and Expanded Access there is a $0 copay for Value Tier 1 providers at a Value Tier 1 location and $5 or $15 copay for all other PCPs or specialists, depending on your coverage. Preferred Primary Care Physicians or Specialists are marked as "Value Tier 1 at this location" in your search results.

$0 copay for Value Tier 1 specialists applies to: Allergy & Immunology, Cardiology, Endocrinology, ENT, Gastroenterology, OB/GYN, Ophthalmology, Orthopedic Surgery, Rheumatology and Urology. You are responsible for a standard office visit copay for any specialties not listed.

Laboratory, X-Ray or High-Cost Imaging Preferred Providers are marked as "Site of Service" in your search results. You are covered 100% for services performed by a Site of Service provider.

You must pay 20% of the rate, known as 20% coinsurance, when you use a Lab/Pathology/Radiology provider that is not listed as Site of Service. When lab and radiology services are performed at a doctor's office, you must pay the applicable office visit copay depending upon the provider's tier status.
