Find Primary Care Physicians, specialists and all other provider types
After selecting your plan’s link below:
- Enter your desired location (zip code) and search criteria for provider type (e.g., Family Practice, Oncology etc.). A list of search results will appear.
- Preferred Providers are indicated by "Value Tier 1 at this location."
Some providers are employed by more than one provider group and as a result may not be preferred at all of their practice locations. Please be sure to check both the provider name and the address to be certain it’s a Preferred Provider location.

Find Preferred Laboratory, X-Ray or High-Cost Imaging Providers
Start by entering your desired location, using the ZIP code, enter the provider type (e.g., laboratory, radiology) or the provider name, and select from the drop down. Click the Recognitions button, select Site of Service, and select Done. Your results will show a list of Site of Service providers. In the List View, you will see the designation, “Site of Service for each Preferred Lab/Pathology/Radiology provider.”
