Ohio Association of Health Plans’ Pinnacle Awards honor advances in personal digital care management, behavioral health treatment

COLUMBUS, OH — Anthem Blue Cross and Blue Shield is the recipient of Ohio Association of Health Plans’ (OAHP) Pinnacle Award, recognizing the outstanding achievements of health plans to navigate the ever-evolving healthcare landscape by implementing pioneering healthcare programs. This accolade underscores Anthem’s unwavering commitment to addressing complex healthcare challenges through their groundbreaking, digital-first concierge care program, accessible via a member-centric app.
 
Anthem’s program is an innovative solution designed to overcome the numerous barriers that members with chronic conditions face in maintaining optimal health, including inflexible schedules, low information access, and poor access to healthcare resources.
 
Concierge care empowers members to track health data and symptoms, link data from personal health devices, set personal goals with customized care plans, receive medication reminders, and access a comprehensive library of curated videos and articles.
 
Dedicated care managers collaborate with members to create personalized care plans aimed at helping them achieve and sustain their health goals. Members engage in real-time, on-demand consultations with care managers who provide guidance, answer questions, and participate in chats. For needs that cannot be addressed online, care managers connect members with appropriate healthcare providers, such as nutritionists, pharmacists, and behavioral health specialists.

The program has had a profound impact, enhancing healthcare accessibility and elevating patient outcomes. The program has yielded significant results, including reductions in outpatient ER use, reductions in admissions for chemotherapy-related side effects, improvements in Crohn's Disease symptoms and flares, lower A1c scores for members with Type 2 diabetes, and reduced anxiety and depression levels for those in the behavioral health program.

Anthem leadership was onsite to receive the Pinnacle Award at OAHP’s Annual Convention & Trade Show in Columbus on September 26th, 2023.
