To be eligible for enrollment, you must be 65 years of age or older.� Anthem Extras Packages or Premium Plus Dental can be purchased with one of our Medicare Supplement plans, but we do not require membership in these plans to enroll.

Anthem Extras Packages are not available to members of Medicare Advantage plans. If you currently have, or are enrolling in, a Medicare Advantage plan, please see our Optional Supplemental Benefit plans, which are similar to Anthem Extras Packages. Please contact your agent or contact us.
