When you enrolled in an Anthem Extras Package or Premium Plus Dental, you made an important decision to complement your Medicare health plan with benefits that can help you maintain good overall health and well-being.  
 
Below are some tips and contact information needed to access your benefits. Please note that depending on the package (or dental plan) you enrolled it, benefits vary.  
 
How to use your dental and vision benefits: 
 
  • Make an appointment with your chosen provider. Please note that you will usually have less out-of-pocket costs from participating providers.
  • Present your ID card at the time of service.
  • Pay any applicable copays or additional charges.

    Use the ProviderFinder and choose the dental or vision network noted on your ID card.

    International Emergency Dental Program 
     
    For information on the International Dental Emergency Program, and a wallet card you can take on your trip, please download our International Dental Emergency Program slick.

    Extra Cleaning for Diabetic Members*** 
     
    Diabetic members may be eligible for an additional dental cleaning at no extra cost. Learn more and enroll.
