Reporting data to improve health equity in Colorado

Colorado law requires us to provide the state with certain data about people, like you, who have a Colorado Option health plan. This only includes data that you choose to share about your race, ethnicity, sexual orientation, gender identity, and disabilities. If you don’t choose to share your data, we don’t provide it to the state.

Colorado uses the data to improve access to healthcare

It helps the state create programs that give everyone equal access to healthcare. One way the state does this is by building care provider networks that make it easier for Coloradans to receive the care they need. Another way is by educating doctors about how to meet the healthcare needs of some people who may experience health inequities because of their race, ethnicity, sexual orientation, gender identity, or abilities.

If you share your data, it will be deidentified for state reporting

We only report what’s called aggregate data. That means we combine data from all of the people who choose to share it, but no names or other personal information are tied to it. Your personal information stays safe and protected.

If you would like us to share your data with the state, please complete My Health Check-in

My Health Check-in is a tool on the SydneySM Health mobile app and anthem.com. When you answer questions about your health, it gives wellness tips and ways to meet your health goals. When you complete a My Health Check-in assessment, we will report the data you share to the state.

My Health Check-in is a wellness tool we offer to everyone who has an Anthem health plan. You don’t have to use the tool if you don’t want to.

If you’d like to use My Health Check-in, find it here.
