When you enrolled in an Anthem Extras Package or Premium Plus Dental, you made an important decision to complement your Medicare health plan with benefits that can help you maintain good overall health and well-being.  
 
Below are some tips and contact information needed to access your benefits. Please note that depending on the package (or dental plan) you enrolled in, benefits vary.  
 
 
How to use your dental and vision benefits: 
 
  • Make an appointment with a provider*
  • Present your ID card at the time of service
  • Pay any applicable copays and/or deductibles

    International Emergency Dental Program

    Extra Cleaning for Diabetic Members
